Services for Jack Leak Cook Jr., 37, of Burleson, will be at 4 p.m. Monday at Chambers Cemetery with the Revs. Tommy Lovett, David Gieger, Harold Nichols and Benny Burlison officiating. Burial is under the direction of Bailey & Foster funeral Home.

Mr. Cook died Thursday

He was born Sept. 5, 1955, in Houston, the son of Helen Fisher Cook and the late Jack L. Cook, Sr.

Mr. Cook is survived by his wife, Pam Cook of Burleson; daughters, Elicia Cook and Andria Cook, both of Burleson; son, Matthew Cook of Burleson; his mother of Palestine; and brothers, Joe Bob Cook of Palestine and Dean Cook of Lolita.
